A pacemaker lead is not just a consumable. It is the conductor through which the device transmits electrical impulses to the heart muscle. If the lead is faulty or incompatible with the generator, pacing does not work — and the patient's life is at risk.
That is precisely why leads are registered as standalone medical devices, not as an "accessory" to the pacemaker. And this creates a whole range of regulatory complexities worth knowing about.
Why leads are registered separately
A lead is a standalone device that can be used with different generator models. It is supplied separately, has its own shelf life, and requires confirmation of compatibility with specific devices.
In the state register of medical devices, you can find entries where pacemaker leads are registered as a separate category — for example, "Instruments for pacing and ablation" with specific models listed.
Risk class and what it means
Pacemaker leads fall under Class 3 (high risk). This is the same class as pacemakers themselves — and logically so: failure of the lead means failure of the entire life-support system.
Registration will require a full range of studies:
-
technical testing — verification of electrical conductivity, insulation, mechanical strength, and resistance to deformation;
-
toxicological studies — biocompatibility of materials contacting heart tissues;
-
clinical trials — confirmation of pacing effectiveness and safety in real conditions.
What has changed in 2026
Since March 1, 2025, new rules for medical device registration have been in force (Government Decree No. 1684). The transitional period has ended, and all applications are now reviewed under the new requirements.
One of the key changes is the registry-based registration model. Paper registration certificates have been abolished: proof of registration is now an entry in the state register.
Additionally, on September 1, 2026, a new conformity assessment procedure came into force (Ministry of Health Order No. 421n), which requires testing organisations to be independent and impartial in relation to the applicant.
For foreign manufacturers, it is important to remember: the deadline for updating authorised representative data has been extended to September 1, 2026 (Decree No. 1206). If this is not done, the registration certificate may be cancelled.
Specifics of lead registration
1. Compatibility with generators. The registration dossier must specify the particular pacemaker models with which the lead is compatible. If the list is expanded or changed, this may require amendments to the registration documents.
2. Product range. Leads may be produced in different sizes — with different lengths, connector shapes, and fixation types. Several models can be included in a single dossier if they share the same intended purpose and principle of operation.
3. Sterility and shelf life. Leads are supplied sterile and have a limited shelf life. The documentation must confirm the sterilisation method and the claimed shelf life.
4. Post-registration modifications. Any changes — in design, materials, configuration, or software — must be analysed for their impact on safety. In some cases, a new registration will be required.
How Elibrium helps with registration
We specialise in registering complex Class 3 medical devices, including pacemaker leads. Our experts:
-
audit your documentation and determine the correct risk class;
-
develop technical and operational documentation taking into account the specifics of leads;
-
select accredited laboratories for technical and toxicological testing;
-
organise clinical trials at centres authorised by Roszdravnadzor;
-
prepare the dossier taking into account generator compatibility and product range;
-
submit the application through the Gosuslugi personal account and support the expert review until the registration certificate is issued.
Important: every modification of the lead or change in the list of compatible generators is a potential reason for refusal if not covered in the dossier. We operate transparently: timelines and costs are fixed in the contract.
Registering pacemaker leads is a strategy where compatibility, product range, and clinical data must all be accounted for. Elibrium offers a route where risks are calculated and the result is guaranteed.
